Service Description

A hospitalization can be stressful and confusing for older adults and their families. Nest Hospital Advocacy provides an experienced care manager to serve as an advocate, coordinator, and trusted partner during a hospital stay. Our goals is to help families understand the plan of care, improve communication among providers, identify discharge needs early, and support a safe transition to the next level of care.
